Get a sick leave certificate (L4) online during a teleconsultation
An electronic sick leave certificate (also known as L4 or e-ZLA) can be issued by any specialist doctor. However, the doctor must verify whether there is justification for issuing the document, so obtaining an e-Sick Leave requires a consultation about your health.

How to get sick leave online?
- Check facilities and doctors offering online consultations with the option of e-Sick Leave
- Book and pay for your e-Visit
- Start the consultation with the doctor by clicking on the link received by email
- If necessary during the e-Visit, share test results or other necessary documents with the doctor (via chat window)
- Discuss your health condition with the doctor online – if justified, the doctor will decide to issue the e-Sick Leave
- Then the doctor will ask for your PESEL – all other data will be retrieved by the system for the e-Sick Leave form (electronic version of L4)
- After the doctor signs it, your Sick Leave will be automatically sent to your employer and ZUS.
What information is needed to get e-Sick Leave?
During the online consultation, the doctor only needs the patient's PESEL number. The rest of the data necessary for issuing the e-Sick Leave will be retrieved from the e-ZLA system. So, you don't have to worry before the visit that the doctor will ask you for a lot of data. It's worth having your ID card on hand if you don't know your PESEL number by heart.
Are there any differences between e-Sick Leave obtained in a stationary office and online consultation?
The sick leave certificate received during a stationary visit does not differ from the one obtained during an e-Consultation. Both authorize an employee to be absent from work to the same extent.
Does the employer see the reason for sick leave?
According to Article 22 of the Labor Code, the employer cannot demand information from the employee about their health. An exception is only when this information is necessary for the employee to exercise specific labor law rights. It follows that the employee is not obliged to inform the employer about the reasons for their illness.
However, in the case of illness, the employee is obliged to inform the employer immediately that they will not be at work. Additionally, they should also inform you about the expected duration of the illness, and not later than the second day of absence from work.
Can I get both e-Sick Leave and e-Prescription during online consultations?
Yes, during online consultations – if justified – the patient can receive both e-Sick Leave and an e-Prescription for the appropriate medications. The fee is only for the online visit itself. On the haloDoctor platform, the patient does not pay an additional fee for issuing an e-Prescription or e-Sick Leave online.
Can a student receive a sick leave certificate during an e-Visit?
If a person studying suddenly falls ill and cannot attend university classes, the doctor during an online visit has the opportunity to issue a certificate of incapacity to participate in classes. It is fully acceptable to higher education institutions in Poland as an excuse for absence from classes.

The length of L4 depends on the diagnosis made by the doctor. Depending on the symptoms, well-being, and, if necessary, morphological or imaging test results, the doctor decides on the duration of incapacity for work. It is worth remembering that sick leave can be extended in case of no improvement or deterioration in the patient's condition.
There is no limitation preventing a doctor from issuing L4 for only one day. If the doctor believes that the employee cannot perform work for health reasons on a specific day, they have the full right to issue sick leave.
The employer does not have the right to obtain information about the employee's illness, and the employee is not obliged to inform them about the specific health issue they are dealing with.
If the sick leave is short-term, related to a passing and transient indisposition of the student, it is possible to obtain sick leave from physical education classes during an online visit. However, if the sick leave is to last longer, or even the entire school year, specialists recommend a visit to a stationary office for a more in-depth medical diagnosis.
If a student falls ill and cannot attend university classes, the doctor during an online visit has the opportunity to issue a certificate of incapacity to participate in classes. The certificate is not in electronic form.
It often happens that a patient seeks advice from a doctor a few days after the onset of symptoms. It's worth remembering that a doctor can issue sick leave up to 3 days retroactively from the date of examination – this is provided by law. Only a psychiatrist can specify a longer retroactive period in sick leave if they consider it justified.
Sick leave due to illness also applies to an employee hired under a contract for a specific task if they voluntarily opted for sickness insurance. If the contractor has chosen this solution, they are entitled to sickness benefits.
